Forms include our newsletter subscription form, Contact Us form, Intake form, and Credit Card form.
Our credit card processing is handled by Square. We do not have access to your credit card information (credit card number, expiration date, etc.) just your billing information (name, address, etc.)
We only use the data collected in these forms for its intended purpose: if you sign up for our newsletter, we’ll only use your email to send you a newsletter, nothing else. If you fill out the contact form with a question, we’ll only use your email to answer that question. The information submitted in the Intake form is required if you have purchased a consultation with blood work, so we can create a lab requisition order for a blood draw.
In all cases, feel free to call the office and submit your information by phone rather than via online forms. We can process credit cards over the phone.
Submitted forms generate an email to our office and a database entry, both containing the information you submit. This means your data is stored in our business email accounts and on online databases. Online databases are run by service providers including Webflow, Ecwid, and FormDr. This data will be stored indefinitely unless you request its deletion.
We do not share data with any 3rd parties, aside from the service providers needed to operate forms, listed above, and Google Analytics, described below.
To request information on what personal data we have stored, or to request its deletion, please contact the webmaster: firstname.lastname@example.org
The shopping cart that updates when you add an item uses localStorage to temporarily store your data. This is so that the site can recognize that you are the visitor that added an item to their cart, even if you go to another page. We do not have access to this data, it is handled by our store software. For additional information, feel free to contact the webmaster: email@example.com
To specifically block Google Analytics cookies, you can download the Google Analytics Opt-Out plugin for your web browser, available here: https://tools.google.com/dlpage/gaoptout
It is available for Microsoft Internet Explorer 11, Google Chrome, Mozilla Firefox, Apple Safari and Opera browsers.
I've been on Testosterone therapy for 2 years and have gone through 3 Doctors all of which failed me either by over supplementation of vitamin D to the point I developed hypercalcemia or over phlebotomizing causing me to become anemic. I was just about to give up and deal with my very low testosterone when I came across a youtube video of Dr. O’Connor.
