Thomas O’Connor MD, PA uses cookies, localStorage, and online forms to provide a functional website experience on testosteronology.com and to track aggregated visitor behavior.

oNLINE fORMS

TYPES OF FORMS:

Forms include our newsletter subscription form, Contact Us form, Intake form, and Credit Card form.

CREDIT CARD FORM:

Our credit card processing is handled by Square. We do not have access to your credit card information (credit card number, expiration date, etc.) just your billing information (name, address, etc.)

USE OF DATA COLLECTED IN FORMS:

We only use the data collected in these forms for its intended purpose: if you sign up for our newsletter, we’ll only use your email to send you a newsletter, nothing else. If you fill out the contact form with a question, we’ll only use your email to answer that question. The information submitted in the Intake form is required if you have purchased a consultation with blood work, so we can create a lab requisition order for a blood draw.

ALTERNATIVES TO FORMS

In all cases, feel free to call the office and submit your information by phone rather than via online forms. We can process credit cards over the phone.

STORAGE OF INFORMATION SUBMITTED IN FORMS

Submitted forms generate an email to our office and a database entry, both containing the information you submit. This means your data is stored in our business email accounts and on online databases. Online databases are run by service providers including Webflow, Ecwid, and FormDr. This data will be stored indefinitely unless you request its deletion.

3RD PARTY DATA SHARING:

We do not share data with any 3rd parties, aside from the service providers needed to operate forms, listed above, and Google Analytics, described below.

COOKIES AND LOCALSTORAGE

LOCALSTORAGE:

The shopping cart that updates when you add an item uses localStorage to temporarily store your data. This is so that the site can recognize that you are the visitor that added an item to their cart, even if you go to another page. COOKIES:

Our website hosting software uses cookies to track visits. We also use Google Analytics to track visits and behavior. This is aggregated data, meaning we can’t link this information to an individual, but we can see that a person in Location A visited the site at 9pm, and viewed the home page, Social Media page, and Contact page. These cookies expire over a variety of time periods, from 10 minutes to 2 years. Our implementation of Google Analytics uses IP address anonymization.

BLOCKING COOKIES:

We have no way of turning off the cookie functionality of our site - it’s built into our website hosting. However, you can adjust your web browser’s preferences to refuse cookies.
